Sam is in an accident that renders him unconscious in the psych ward for a few days. Dean visits him but someone else has caught his attention: a quiet boy with a tragic past. Dean ultimately befriends this strange boy with hopes of getting him to speak and live easier.

He is definitely not expecting the events that unfold.
