It has been two and a half years on from when Imori bled out in Sakura's arms after being killed by his brother Kuma. Sakura had been distraught trying to cope with his loss, and demonising the actions of Kuma, who Imori loved like a brother, for his actions. 

But life moved on without Imori, and Sakura loathed it. She hated that Kuma had moved on as if he hadn't killed the closest thing to family he ever had, continuing to work under control of 'the Hive.'

So when she ends up at Sunagakure to save the Kazekage, Sakura runs into a masked group and hears the voice of a boy she thought was dead. And after that, the teenager begins to pop up all over the country with an Organisation that were arguably severely worse than the Hive.
