Izuku as the brother of Ryuko Tatsuma aka Ryukyu. He'll have a quirk called "Azure Dragon". It'll be essentially Kaido from One Piece. It will be a quirk and not a Devil Fruit.

Ryukyu is her canon age, which is only about 10 years older than Izuku. Miruko will be slightly aged down, but still a pro hero. She'll be 20 when Izuku is 16. UA starts at 16 Years of age in this au. 
Izuku will know Miruko since childhood, and will know that she and Ryuko are Pro Heroes. The two start dating a month before the UA entrance exam.
Izuku and Ryuko's parents will be dead, meaning Ryuko raised Izuku since he was 6 and she was 16. She had help from her friend Rumi Usagiyama aka Mirko.
