While Sherlock was in Serbia, after he was caught, and before Mycroft arrived, Sherlock was hallucinating/dreaming/imaging that John is with him looking after him, giving him tender kisses, gentle touches or possibly simply taking advantage (in a very consensual way) of the fact he is chained up.
