Tony had been in a coma for 2 months. Steve went back in time to place the stones back to the timeline and places that they came from.
When Steve went to Vormir, he met Red Skull and started to fight him, when he had his shield on Red Skulls throat, ready to give him one more lethal hit with his shield, Red Skull told him that he was the guardian of the soul stone. Once Steve heard that he quickly got away from him and told him that he wanted to return the soul stone as he handed it to Red Skull. Red Skull took it and suddenly a large beam of light appeared in the dark and cloudy sky.
All of the sudden the stone disappeared from Red Skulls hand and Natasha appeared in front of Steve. Steve took her with him and returned the rest of the stones.
