Zenitsu had a crush on Tanjiro since when they were kids and Nezuko knows to when they turn into 18 and Tanjiro is 19 Zenitsu he always had a sick so he go to hospital to check up and that day he had a cancer he told to Nezuko about his condition but suddenly Nezuko accidentally said to Tanjiro that zenitsu had a crushed on him and had a cancer, so that day Tanjiro he faked he's feeling to Zenitsu not to broke his heart to made him happy (Tanjiro had a crushed on Kanao)
