Cassandra has a very rare condition, that only happens once every 400 years when a child is born on May 13th. Cassandra's main heart is extremely fragile.
Basically, Cass has 3 sections of her heart: her family/friends, her health, and the things she loves. If one of those things dies or disappears, her heart will begin to Crack.
In the case of the post-apocalyptic Minecraft world, after seeing her family die one by one, the family section of her heart slowly began breaking. This hurt really bad and it bothered her, but she never told anyone.
If she told her family, then she had a feeling they would go out and try to find a cure, which would be life-threatening because of the Dreadlord and Null.
She didn't want to lose anymore family members, so she kept it a secret. Unfortunately she still lost them, which damaged her heart even more.
And when she lost Steve and Jinxi, the last two remaining members of her family, her family section nearly shattered.
Luckily, when she united with the past versions of the Minecraft family, her heart began to heal. But just because her heart has begun to heal doesn't mean that her heart can't break again.
This is actually the same condition I made up for my main oc, Wendi Davis, and I felt like it would be good to add it to Cassandra
