Some definitions:

On call = not what it sounds like, essentially it's just an extra shift. You don't go home, or rest, or wait for anyone to call you. It's literally just more working.

Intern = the first two years out of medical school, we work for the government hospital system, rotating through different disciples, to gain experience before we are allowed to practice medicine alone.

Free time = yeah, I have no idea about this one.

My first year entails the following disciples, each for three months: internal medicine, surgery, obstetrics and gynecology, and paediatrics. I started with internal medicine.

Internal medicine = sick people, really; cardiology, neurology, lung medicine, gut medicine.
