For starters, Deku already gets hurt from his quirk by breaking his bones.
The main way I seen his quirk malfunctioned is possession. The past users of One For All takes over Deku's body and makes Deku kill himself. I honestly could see the first part happening but the second part doesn't really fit. But at the same time, I don't know what they would do.
I hope this doesn't happen in the show, because it would be scary. Just imagine them training and Deku out of nowhere does this:
