This ending is a bit tricky but easy to get. I got this during my 2nd run. Well precisely I load files on my 1st run and accidentally get this ending.
Make sure you pass the 1st month (usually you can get either the 1 or 3 ending on the 1st month). I get this by visit every place (until got molest n stuff but not focus on hunt) but not regularly and on the last day you can either 'REST' or '???' or visit places IF you never visit the place (it will trigger another ending once you have sex/ visit too much). Make sure you pass the 1st month to trigger this. Just keep your relationship with NPCs and the twins at minimum rate.

When Nero paid off the debt, BOSS will awake him and Nero realize he's been hallucinating Dante and Vergil all along. He admits by saying he misses Dante and Vergil so he kept playing/hallucinate them being there with him. "I play this game and I lost..."
This is a sad ending to me because he just wants to be with his family that he finally found.
