Natsuki = her "father" is a creation that her mind made up, this "father" isn't real and has been dead. When her father died she was sent to the mental hospital, because of her hallucinations. Has eating disorder.

Monika = Saw a vision of the world, and has been trying to tell her doctors that the world isn't real, it's a game. Mental hospital.
