Madness is the side-effect of regular people coming in contact with a god.
The frenzied flame and greater will were said to be part of the same entity at some point of time.

Then something caused the greater will to seperate from it, and thus life began form the primordial space juice.

(That world's version on the big bang)

The frenzied flame wishes to burn everything and return to that primordial state again.

The golden order feared it, and tried to keep it away. Ironically, they summoned the frenzied flame when they accused the merchants of blasphemy and buried them alive.

The madness is caused when the frenzied flame enters the body of someone unworthy.

They are either killed, or go mad from the immense knowledge (like what happens if someone sees Cthuhlu)
The only person know to have not instantly mad is our tarnished.
There are others who can resist a little, but they end up in a half mad state (like Vyke).
Shabriri isn't effected too much, because he's practically mad already.

When our tarnished defeats the Elden Beast, the flame is able to fully manifest, and the true power of the frenzied flame burns down the lands between.
