I'm give y'all more of a reasoning for why I have the headcanon
I like to think that Bryan can't heal anything properly if he's tired or exhausted and that he can't heal scars that already healed so if he did try to nothing would happen
I really hope you guys are understanding this
So when mario defeated bryan you could see his healing magic (it was probably just for effects) but he was beaten up pretty badly and he was tired so the wound he was trying to heal didn't heal right and left a permanent scar on his face
