During the entire book/movie, Hazel was in the hospital. Augustus Waters was purely justa hallucination caused by a lack of oxygen to her brain. When the doctors were finally able to provide her body with enough oxygen, Augustus disappeared, this causing his death.
This explains why her time spent with Augustus was so perfect, likea dream. In a way, you could say that Augustus was somewhat of an angel, created by Hazel Grace's mind. She was the one who truly feared oblivion.
