when you turn eighteen, time is warped and you are taken to another world with only you and your soulmate to hang out for two years. time is slowed in the real world to ensure you are eighteen when you return, however when the two years are over, all memories of their time together will be foggy and hard to distinguish, which will make it difficult (sometimes impossible) for them to find each other again.

(note that george and dream both are the same age and have the same birthday for plot reasons)
