You are playing as Natasha Hart, the Oldest Sibling In the Hart Family. She has fallen into a coma after an accident just like falling into a sewer system. You run from your anxiety and the creatures who have come to take you away from the life you have. You end up in a world where some inanimate objects are alive which represent how your anxiety makes you feel judged by everything even things that aren't living. You meet your deceased brother Briley, he died nearly a decade ago and he still looks the age he died. He follows you everywhere and even back into the sewer system. He wants to help you out and let you live the life he could never have lived. Even if he was dead you still did not know he didn't need protecting anymore.
It's a representation of how the fight goes in a coma. It's you who fights and your family (even the dead) are there to support the war you're having to survive.
