If his cartridge gets damaged while he's still inside it, he becomes seriously injured and once he gets hurt to the point where he's borderline unconscious, he's forced out of the cartridge and requires medical attention immediately.
If his cartridge gets damaged while he's outside of it, his body starts glitching out and - this only happens when irreversible damage occurs - he starts coughing up water and blood, which is a pretty painful thing to endure.
In other words, damage the game, damage the ghost.
