Yn's POV
"Almost halfway done. Are you holding up" I ask Will. "I'm sure that runner's high is going to kick in any second now. That or heatstroke" Will said. "Yeah. It's not the best day for a heat wave" I say. "Why'd you want to do this marathon again" Will asked me. "In a few months, I'm going to start gaining tons of weight, so I have to get fit while I can. So, I was thinking maybe later we can do something else" I say. "What? Like collapse" Will asked. "No. Maybe a bedroom activity. Or kitchen table. Shower" I say. "Let's go, Doc. Pick up the pace" Will said and started running ahead of me.
During the run, we got a call from April, who was apart of the medic team for the marathon. She said that her and Noah had a guy named Ignacio who badly injured, and she needed us right away. When we got to them, Noah was attempting to intubate the man and there was a a chest tube in, but there was blood everywhere. "What the hell are you doing" Will asked. "I'm trying to intubate. He had a massive hemothorax, and I had to put in a chest tube" Noah responded. "No, what you had to do was wait" Will said. "Dr. Halstead, this man would have died" April said. "Yeah, judging by all the blood, if we don't get him into surgery in the next few minutes, he will anyway" Will said.
"Any ETA on transport" I ask. "They're still saying it'll be a while" April said. "Well, we could open his chest and try to address the bleed. It might buy him enough time for the ambulance to show up" I say. "Yeah, it might, but there's no guarantee I could even find it" Will said. "The only guarantee is he'll die if we don't try" I say. "No, I've been down this road before trying extreme measures. It doesn't end well" Will said. "His pressure is down. Trachea feels deviated to the right" April said. Will sighed, and looked up to meet my eyes. "I need a scalpel" he said.
As soon as April have Will a scalpel, he cut into Ignacio's side. "Retract his ribs" Will told Noah and I. I grabbed one side of the skin and Noah grabbed the other, both of us stretching as much as we could. "Anything" I ask Will. "Nothing but clots. I don't see it. I've got to get behind the lungs" Will said. "He still has a carotid pulse" April said. "Oh, man. A rib perforated the aorta" Will said. "Can you clamp it" Noah asked. "With what? My hand" Will asked. "It'll buy him 20 minutes" I say. "Any longer there's a good chance he'll be paralyzed" Will said. "If you don't, he'll bleed out" I say.
Will sighed but stuck his arm deeper into Ignacio's side, feeling around for the hole. "I've got it. Now he needs more blood and a miracle, which could happen if we get him on portable bypass. They've been doing it in France. We have a unit in clinical trial at Med. April, call Maggie" Will said. "On it" Maggie replied. "We've got about 19 minutes to get Ignacio hooked up before he's paralyzed or worse" Will said.
.........................................
"There's blood in the tube. Saturation's 75%" April said. "It must be a pulmonary laceration. We've got to pack his lung and get the blood out of that tube before he drowns" Will said. "You want to pack the chest, and I'll suction the tube" Noah asked Will. "With what free hand" Will asked. "I'll suction the tube" April said. "I'll pack the chest. Noah, grab both sides, and try to keep them open" I say and let go of my side of skin. "My wrist is cramped up. If you let go on the ribs, I'm done" Will said to Noah.
I could tell Noah was getting tired, and to be honest, I wasn't feeling so well myself. Running half of the marathon and then using the rest of my strength to retract a man's ribs left me with no energy. "It's closing on my wrist. I can't hold this" Will said. "Come on, Noah, pull" I say. "Guys, I'm here. Sorry it took so long" Jeff said and ran over to us with the supplies we needed. Oh, man. I need two clamps. How long's it been" Will asked. "17 minutes" April replied. "Three to go. Hurry up" Will said to Jeff.
We got the portable bypass machine hooked up as fast as we could, but by then, it had been 24 minutes. At that moment, my lack of energy hit me. "Will, I'm not feeling to good" I say before collapsing into his arms. "Yn! Yn" Will shouted and set me down gently on the ground. My vision started getting fuzzy, and after a few seconds, I passed out.
.........................................
When I woke up, I was in a hospital bed. There was an IV in my arm, and a bag of saline hanging up next to me. Someone was clutching my hand, and I looked over to see Will. "Will" I mumble softly. He looked up, and when he met my gaze, he smiled. "Yn, hey. How are you feeling" he asked. "Fine. What happened" I ask. "You passed out from dehydration" Will said. "Oh my god. Is the baby okay" I ask. "The baby is fine. Everything is fine. Once you finish this bag of saline, I've got a surprise for you" Will said.
Once I finished the bag of saline, Will took me back to where we had to cut our marathon short, and we began to walk the rest of the way. After a little bit, we came upon the finish line. "A few more feet, you can officially check marathon off your bucket list" Will said. "'Officially' is a bit of a stretch" I say. "Hold that thought" Will said and ran over, picking up the finish line from the ground, holding it up for me. "It doesn't get more official than this." I smiled and ran over, passing the finish line and jumping into Will's arms. "With a time of ten hours and six minutes, Dr. Yn Ln has just set a new marathon record" Will said and kissed me passionately. "You are the best boyfriend one could ever ask for" I say. "Thank you" Will said. "You're also going to be the best father to our baby" I say. "I love you" Will said to me. "I love you too, Will."
