He was taken by Palpatine to the Imperial Rehabilitation Center on Corcusant, where he was restored by using metal body parts and a ventilator system with helmet to survive, as he was burned and maimed as a result of fighting Obi-Wan on Mustafar, and they went to a deserted planet to oversee the creation of their new weapon, the Death Star.
