Tsukishima died from a car crash. Yamaguchi and him were dating and yamaguchi loved him dearly.
Yamaguchi could see a bright future coming for tsukishima and was really upset that it had to end so soon. He just couldn't stand the fact that his soulmate left him so suddenly especially since yamaguchi grew up with tsukishima and had loved and admired him for as long as he could remember.
It was nearly time for his funeral and he brought a bear that tsukishima had won for him at the arcade several weeks ago. He placed the bear at tsukki's gravestone and cried continuously. He began to sing to him. Listen to the song- it's at the top above the title.

Everyone else had left and he said 3 simple words which tsukishimas soul up in heaven reacted to
"I-i m-miss y-you" he kept saying it as he sniffled.
He then said a few more words
"I'll never forget about you, tsukki... I'll miss you..."
