Just fill out the form. But first, I have a few conditions yet again.
- Serious requests only. If you are doing this just to mess with me, believe me when I say this you'll have hell to pay.
- Three cover requests per individual.
- The fore-started condition can be overlooked if you are working through a series.
- Put your form in the comments to this chapter alone and no other.
- If you like one of the posted book cover and want something similar done put your form in the comments part of that chapter.
- Fill out the complete form.
- Put your whole form in a single comment.
