I've heard of malevolent, and I really like that word, but lately, it seems like a very Disney-esque word if that makes sense. For some reason I can't pinpoint, it has a fairytale connotation for me. Anyway, this can be a good alternative. If you're writing a story, possibly a fantasy, I could see the second two definitions being used as well, if there's a medical side to your plot. I don't know. Seriously though, say this word aloud; it sounds cool and serious and amazing.
