Blood players are always aware of their own heartbeat and the hearbeats of those that surround them.
After the meteor massacre and the ensuing chaos that led them to the three year journey, Karkat realized that everything around him was a lot quieter than it used to be.
It only got worse meeting everyone in the dream bubbles because what should have been a cacophony of sound was silent except for his own heart.
