Step 1: Open a canvas of dimensions 500x500.

Step 2: Open this texture as a layer and resize it according to your liking-

Also change the blend mode to hardlight

Step 4: Then, open up a PNG of your model and place him/her in the middle. Add the following layer style-

Step 5: Next, type in your text. I used the font 'Matilda' for the name 'Allie'. Change the font colour to d44246 or something similar (I've lost the colour code of the original colour I used). Then, add the following layer styles-

Step 6: Rasterize the layer and go to Filter - Scanlines and set the scanline intensity to 45

Step 6: Rasterize the layer and go to Filter - Scanlines and set the scanline intensity to 45. (or any amount you like).

Step 7: Create a new layer and using the rectangular shape tool, create a small long white box and place it over the model's eyes. Write your text over it in a red hue using Arial or Arimo (all caps, bold and italicized).

Step 8: As I've lost the original lighting texture I used to make this, I suggest you guys choose any lighting texture that you already have and set the mode and opacity to your liking. 

And that's it! You're done! :D
