This was suggested by @Pikachu904726 with the scene from SAO, Asuna's death.
So when I was thinking about that pose, I wanted to find a situation where Mordred was shocked and broken when she learned about Mikoto's state. This lead to an idea where Mordred was Mikoto's Servant instead of Saber, when Saber rescued Mikoto after the fight.
However in this situation, Mikoto received the injuries that Mordred did after she turned back to normal, and Mordred felt like she failed to protect Mikoto from her own promise.
Though I had to admit, I know Mikoto wouldn't let Mordred wear that kind of clothing if that was the case. 😅
