I wrote in an old analysis about how Mika's smile seemed kind of sad in the chapter two art.

Now, I feel like there is another way to interpret Mika's smile, given the chapter 34 art is similar to the chapter two art.

One possibility is that in Yuu's memories of Sanguinem, Mika always smiled. In this art, Yuu may be imagining grown-up Mika smiling at him. At this point in the manga, Yuu and Mika did not have the time to talk during the first time they reunited, so Yuu may not know Mika is moody and unsmiling these days.

It can be unsettling to see Mika smiling with blood on his face, not to mention a shadow covers his face. I think the reason why Yamamoto drew this in such a way was to make readers a little suspicious of Mika (early on in the series). Maybe for the suspense. Who knows?

Mika seemed unaffected after breaking a female soldier's wrist and killing her, even smiling after a few panels as he was thinking of Yuu.

He apathetically sliced down an entire helicopter full of vampires and smiled afterwards with blood on his face, thinking of Yuu.

He did not try to save Shinoa squad when they were bitten by vampires during the first reunion with Yuu. He does not trust the people Yuu trusts. He fought on the same side of the battlefield as Ferid. The only thing he seems obsessed about is taking Yuu and running off somewhere far away, even if it is against Yuu's will. He had done one or two good things for other people while he was a vampire before chapter 34, but..
