Like I said in the comment section, Ah Reum died on the surgery bed, she didn't survived the surgery so Jungwon actually was imagining that he's the one who died and gives Ah Reum his heart, he cannot believe that Ah Reum is actually give up her life in that surgery bed so Jungwon now spending his days imagining/thinking about Ah Reum .
