Helen's parents wanted a girl, so when Helen was born they dressed him up as a girl and gave him a girl's name. It wasn't until Helen was 6 years old and started attending school that they finally treated him like a son and gave him boy's clothes to avoid attention. This emotional distress built up over time and eventually made what we see today.
