To help this make sense for those of you who haven't read KotLC, Elwin is the doctor in the Lost Cities.
(SPOILERS) Sophie is referring to Physic, the Black Swan's doctor. The reason they would need to sneak into the Lost Cities is because they were sort of banished. (SPOILERS OVER)
I just love Keefe's solutions to problems. If the doctor isn't good enough, the best thing to do is kidnap the doctor you know is good.
