Stefan dated Elena only briefly, but he fell in love with one of her best friends, June. June Cheon was the nice girl, the one with a wide smile, the one that always helped, and the one who always controlled her anger. She had always been a good girl, until she began to fall in love with Stefan Salvatore. It was slow, beginning with her wide smiles faltering, deciding not to help him, and her anger getting out of control when he did something foolish.
When Damon gets bitten by a werewolf and Stefan sacrifices his freedom to Klaus, the younger Salvatore finally had the guts to tell her how she feels. So, he calls her while walking back, wanting to use the time of freedom he has left to finally give her that kiss. She answers, and they begin to talk, and then her voice is cut halfway. The sound of metal crashing together is fresh in Stefan's mind, haunting him.
When Elena goes to Chicago with Damon to get the youngest Salvatore back, she confronts him about it, blaming him for June's death. It's what causes him to completely break down.
June can come back a vampire or just leave her dead, whichever fits the plot best in your mind.
