Phantom Mangle isn't a physical animatronic. He is a hallucination of the night guard caused by the ventilation in the attraction. He is a ghostly version of Mangle, he is green with dark eyes and looks like he was burned. Interestingly, Phantom Mangle has never been called a female, and since this isn't the REAL Mangle, and is instead a hallucination, let's just go with, the night guard thought Mangle was male. (Not serious). For some reason, Mangle was counted as a "secret character" in the early days of FNAF, even though all the phantoms are technically secret, but I can assume it was because Mangle is for some reason not in the extras menu.

Phantom Mangle appears on Night 2. It cannot jumpscare you, instead it appears on a camera, and if you stare at it for more than a second, loud radio noises will start to go off and cause your audio to be disabled, and Mangle will appear at your window temporarily.

Ultimate Custom Night:

Phantom Mangle returns for the ultimate custom night and is listed as the 16th character on the character select screen. As mentioned before, Phantom Mangle has never been referred to as female, he is not in any of the ladies night, and his description only says, "he" on it. In this game, he will work the same as Phantom BB, he will appear on a random camera, and its easy to spot him as he is now a bright green this time around, instead of being fully black and being a "Shadow Mangle." Sometimes he and BB are on a screen at the same time. If you stare at him for too long, he will appear in your office on the side and cause loud noises, which can attract characters like Music Man who hate loud noise.

Phantom Mangle appears in the FNAF 3 section of this game. This time around however, he actually has a jumpscare.
