I loved the way the title was incorporated into the cover, like how "Verse" was underlined with the book. I also loved the overall aesthetic of the stars.

Title: 7/10

Your title is very poetic and fits extremely well, especially after reading the blurb. However, it doesn't really fit your book. The chapters in your book don't follow one specific story, and instead narrate multiple, different stories. Maybe it'd be better to change your title to Stories Told in Verse.

Blurb/Description: 8/10

I LOVED THE BLURB!!! It was so touching, and really fit the overall theme of your book. However, you listed too many genres and it ruined the flow.

Creativity and originality: 6/10

There were certain poems that I really enjoyed. I especially loved "The Girl" and "Perfectly Imperfect." I loved how haunting "The Girl" was, and "Perfectly Imperfect" really resonated with me. It is quite evident that these poems came from the deepest parts of your soul. Some of the poems, however–like the first one–weren't as descriptive. They more or less described actions rather than feelings. I think I would have liked it more if you conveyed more emotion in these poems instead of just the actions. In "Listen," I loved the concept and rhyming. In "Things to Remember," I felt no emotion when reading it whatsoever. The theme of the poem is really overused and cliché, especially since you didn't talk about the emotions being felt.

Writing style, Grammar, spellings, etc.: 4/10

Your writing style is inconsistent. Some of your poems felt less inspired than others. It is quite obvious that you're an amazing writer, but your first two poems weren't that good. The flow felt awkward and the words didn't invoke any strong emotion from me, as a reader. However, your rhyming game is on point! It's amazing.

Overall, your poems are amazing!! Most of your concepts and themes were very interesting and your writing style is very good. While some of your poems felt plain and uninspired, I certainly loved the other ones. Keep writing! You're super talented!

Tips 

Maybe be more descriptive when using imagery and apply synesthesia. It's quite an interesting poetic device. Also, when you write, be authentic. The poems that you made on the fly, like "Listen," were so much better than the ones inspired by your characters.
